Introduction
This Privacy Policy explains how Neocpm handles information when you use neocpm.com, request an appointment, write to the care office, join care notes, or unsubscribe. It is written for patients, families, and readers of the journal. It sits alongside our Terms & Services.
We designed the site to collect little. There are no user accounts, no public telephone directory, and no street address. The information we hold is mostly what you type into a form and the emails that follow. Clinical records created after a confirmed visit are kept as part of care, under the duties that apply to health information, and are described below in plain language.
If you only browse, we do not ask for your name. If you write to us, we need enough to reply and to route a health question safely.
Information we collect
Identity and contact details you submit: your name and email address. We do not ask for a phone number or a postal address on these forms.
Appointment details: department, preferred date, visit type, any pricing plan you select, and the description you write of your concern. That description may include symptoms, medicines, and history. Please include only what the coordinator needs.
Contact messages: the topic you choose and the body of the message.
Email-list details: the address you subscribe, the topics you tick on the subscribe page, and the reason you optionally give when you unsubscribe.
Technical basics: like most websites, the server that hosts these pages may briefly log an IP address, browser type, and the page requested in order to keep the site secure and to understand which pages fail to load. We do not use that log to build a marketing profile, and we do not sell it.
We do not knowingly collect payment card numbers through this website. Fee confirmation, when it happens, is arranged in the email conversation that follows a request, using a method stated at that time.
How we use information
We use form details to reply, to suggest a clinician type, to confirm or decline a visit, to explain a fee, and to send the care notes you asked for. We use them to keep a record of what you asked and what we answered, so a later message is not handled as if we had never spoken.
We use health information you volunteer to prepare a clinician, to decide whether the request is in scope, and to avoid asking you to repeat the same story. We do not use journal comments as a clinical inbox. Comments are moderated as website feedback only.
We may use aggregated, non-identifying counts — for example, how many appointment forms were opened — to improve the pages. Those counts do not name you.
We do not use your information for automated decisions that produce legal effects. A coordinator reads appointment requests. A form’s browser-side validation only checks that fields look complete before the thank-you message is shown. It does not diagnose.
Health information
Symptoms, medicines, photographs of a lesion, and similar details are health information even when you type them into an ordinary form. We treat them with tighter care than a newsletter address. Access inside Neocpm is limited to coordinators and clinicians who need the information for the request you made, and to people who maintain the security of those mailboxes.
Where a law that applies to health information sets a higher standard — consent, purpose limitation, or a right of access — we follow that standard. This policy does not try to rename those laws. It tells you, in practice, what we do: we collect health information for care you requested, we do not sell it, and we do not use it as content for advertising.
Family Circle plans may involve more than one person. We will not discuss one adult’s clinical details with another adult on the plan unless the person concerned has agreed or a law allows it. Please do not put a relative’s full history in a form unless you are authorised to do so.
Children’s information is covered in its own section. A parent or guardian may request a visit for a child and should say so in the message.
Appointments and email replies
Because this website publishes no telephone line, the reply path is email. Email is convenient and it is not a perfect vault. Messages can be misaddressed if a spelling error is made, and they can sit in an inbox other people can open. Use an email account you control. Do not forward a clinical reply to a group thread if you want it kept narrow.
A coordinator will not confirm a visit to a different address from the one you submitted unless we can match the request. If you need to change the address, write from the original one and name the new one.
Suggested times are proposals until you accept them. Declining a time does not delete the fact that you asked. If you want the request closed without a visit, say so and we will mark it closed, keeping only what we must for the record of the contact.
Care notes, subscribe, and unsubscribe
If you subscribe, we store your email, the topics you selected, and the date you joined, so we know what you asked to receive. Notes are occasional. They may include a link back to a journal article, a change in Saturday-to-Wednesday hours, or a prevention theme. They are not a vehicle for selling address lists.
The subscribe form is validated in your browser. You must give a real email format, pick at least one topic, and confirm you want the mail. The on-screen thank-you line means the page accepted the entry. Keep an eye on the inbox you named, including junk folders, for the first note.
Unsubscribe is as available as subscribe. The unsubscribe page asks for the same email and a confirmation. A reason is optional and helps us see whether a topic was wrong, the mail was too frequent, or you simply no longer want it. You may also use the unsubscribe link inside a note. We aim to stop further notes promptly. A note already queued may still arrive once.
Unsubscribing does not delete an appointment record and does not cancel a confirmed visit. Send that request separately to info@neocpm.com.
Cookies and local storage
The pages are static. They do not set an advertising cookie and they do not embed a third-party tracker. Your browser may store nothing from us beyond what it normally keeps in its own history.
Fonts may be requested from a font host so the design can use the Poppins typeface. That request reveals to the font host that a browser loaded a Neocpm page, which is ordinary for webfonts. If you block that request, the site still works and falls back to a system typeface.
Share buttons do not load a social network until you choose to open one. When you do, that network receives the page address you asked to share, under its own policy.
We do not respond to “do not track” in a special way because we are not tracking you across other sites. If we later add a non-essential cookie, we will update this section and ask where the law requires a choice.
When we share information
We share information with a clinician inside Neocpm when they are the person who would see you, and with a laboratory or imaging partner only if you and the clinician agree that a test should be arranged. We share with a technical host to the extent the host stores the email or the site so the service can function. Those helpers may process data only on our instructions for this purpose.
We may disclose information if the law requires it, if it is necessary to protect someone’s life, or to establish or defend a legal claim. We do not sell personal information. We do not exchange it for advertising audiences. We do not allow a data broker to mine the appointment form.
If a business that runs this website were ever reorganised, records would transfer only with the same duties attached, and people with active requests would be told by email where a change affects who holds their information.
How long we keep information
Appointment and contact messages are kept long enough to handle the request, to document what was agreed, and to meet the retention period that applies to health records once a clinical relationship exists. A casual enquiry that never becomes a visit is kept for a shorter administrative period and then deleted or de-identified, unless a law tells us to hold it.
Subscribe records are kept while you remain on the list and for a short period afterwards so we can show that an unsubscribe was honoured. Server security logs are kept briefly and then overwritten.
Journal comments, if retained, are kept as website correspondence, not as part of a medical chart. Do not rely on a comment as a record of your care.
When a retention period ends, we delete the content or strip the identifiers. Backups may linger for a short technical cycle before they age out. We do not use aged backups as a working mailbox.
Security
We limit who can read clinical email, use reputable hosting, and avoid collecting data we do not need. No method of sending information on the internet is perfectly secure. You help by using a unique inbox, by not posting symptoms in a comment, and by checking that you are on neocpm.com before you type.
The forms on this website validate in your browser and then show a confirmation line. Treat that line as acknowledgement that the page accepted the fields. For anything you must be certain a person received, watch for the human reply from the care office. If a reply does not come within the next open clinic window, write again to info@neocpm.com and mention the date of the first note.
Please tell us promptly at info@neocpm.com if you believe an email about your care reached the wrong person because of an address you submitted, so we can stop using it.
Your choices and requests
You may ask what appointment or contact information we hold about you, ask for a correction, or ask us to close an open request. Write to info@neocpm.com from the address you used, and describe the request clearly. We may need to confirm that the address belongs to the same person before we release clinical detail, so that we do not send a record to an impersonator.
You may object to care notes at any time by unsubscribing. You may object to other contact by saying so. We will still keep what the law requires us to keep, and we will still reply if there is an active clinical safety issue that we are already responsible for.
If you believe we have handled information unfairly, contact us first so we can put it right. You may also complain to a data-protection or health-privacy authority that has jurisdiction over you. This policy does not name a single national office, because readers reach us from more than one legal setting. We will cooperate with a lawful request from a competent authority.
Children
The journal is written for adult readers. We do not knowingly collect personal information from a child through the subscribe form. A parent or legal guardian may request care for a child through the appointment form and should identify themselves and the child in the message.
If we learn that a child submitted a subscribe or comment form, we will delete that entry. Clinical information supplied by a guardian for a real care request is kept under the health-record rules, not deleted merely because the patient is young.
Information and location
You may write to us from wherever you are. The website does not offer a located clinic directory, a map, or a street address, and it does not claim a single national base in its pages. Email you send is handled by the Neocpm care office and the hosts that support it. Those systems may sit in a different place from you.
When that happens, we take steps so the information remains protected to a standard consistent with this policy and with health-privacy duties that apply to the record. Do not send information if you are unwilling for it to be handled by email in that way. You can choose not to use the forms.
Changes to this policy
We will update this page when our practices change. The date at the top will move with the text. If a change materially affects how we use health information already collected, we will also say so in a care note or in a reply to people with an open request, where we have an email address to use.
The policy you can read on the site is the current one. Older appointments keep the privacy commitments we stated when they were confirmed, unless a new law requires a different minimum and we tell you.
How to contact us about privacy
Privacy questions, access requests, corrections, and complaints go to info@neocpm.com. Put “Privacy” in the subject line. Describe what you want without repeating an entire medical history if the request is only administrative.
We do not offer a postal address or a telephone number for privacy requests on this website. Email is the channel. We will reply to the address you write from after we are satisfied it matches the person the record concerns. Please allow until the next open clinic window, Saturday to Wednesday, 8:00 to 4:00, for a human reply.
