Agreement to these terms
These Terms & Services govern your use of the Neocpm website at neocpm.com, including the journal, clinician profiles, pricing descriptions, appointment requests, and email lists. By using the site or sending a form, you agree to these terms and to the Privacy Policy linked from the footer.
If you do not agree, please do not use the forms or the email list. You may still read the public pages. We may refuse a request that these terms do not allow us to handle safely, including any request that asks the website to act as an emergency service.
Who we are
Neocpm provides clinical organisation for preventive visits, specialty opinions, and follow-up. The website explains that work and lets you ask for a coordinator to write back. It is not itself a medical device, a diagnostic tool, or a promise that a particular clinician is available on a particular day.
References to departments — cardiology, dermatology, orthopaedics, neurology, surgery counselling, internal medicine, and family medicine — describe areas of practice. They do not guarantee that every treatment existing in that specialty is offered.
The website is not for emergencies
Do not use Neocpm forms, journal comments, or the email list to seek help for a medical emergency. Chest pain, trouble breathing, one-sided weakness, sudden inability to speak, heavy bleeding, a seizure, a serious allergic reaction, or thoughts of harming yourself require immediate local emergency services.
Messages are read during care hours: Saturday to Wednesday, 8:00 to 4:00, with Thursday and Friday held for sessions already planned, and Sunday closed for routine visits. A delay in email is not a failure of emergency care because this channel was never that service.
Appointment requests
Submitting the appointment form sends the details you typed to be reviewed by a coordinator. It does not reserve a clinician, a room, or a time. A visit exists only when we confirm it by email. If we cannot match the request, we will say so and, where we can, suggest a more suitable department.
You agree that the information you send is accurate to the best of your knowledge, including your name, email address, and the history you choose to share. Incomplete or misleading clinical information can lead to the wrong preparation. You may correct a request by writing to info@neocpm.com with the same email address you used on the form.
We may decline or postpone a request that sits outside our scope, that needs emergency care, or that we cannot staff safely. Decline is not a comment on the importance of your symptoms. It is a limit of this service.
No clinician–patient relationship from browsing
Reading a profile, a journal note, or a pricing card does not create a clinician–patient relationship. That relationship begins only when a visit is confirmed and you attend, or when a clinician explicitly accepts an advice arrangement in writing.
Journal articles are education for a general reader. They use examples so that a future visit is easier to prepare for. They are not personalised advice, and comments posted under an article are not reviewed as clinical messages. Do not include sensitive health details in a public-style comment form. Use the appointment or contact form instead.
Health information you choose to send
Appointment and contact forms ask you to describe a concern. That description can be health information. You choose how much to write. We ask for enough to route the message to the right clinician type, not for a full record on the first email.
Please do not send photographs of other people, identity documents, or records you do not have permission to share. If you send an image of a skin lesion or a document, you confirm you have the right to send it and that you understand email is being used because you asked us to reply that way.
We handle that information as described in the Privacy Policy: to respond, to arrange care you requested, to keep a record of what was agreed, and to meet legal duties that apply to health information. We do not sell it.
Pricing plans and fees
Visit Essentials, Continuing Care, and Family Circle are described on the pricing page so you can compare coordination styles. Figures shown there are planning prices. A coordinator confirms the fee that applies to your request before you are asked to commit. Laboratory work, imaging, and procedures are quoted separately when they are recommended.
Monthly plans renew only by the arrangement confirmed in email. You may ask to change or stop a plan by writing to info@neocpm.com before the next period begins. We confirm the change in writing. Unused visits are not exchanged for cash. If a clinician cancels a confirmed visit, we offer another time or an adjustment of the fee for that visit.
Nothing on the pricing page is an insurance policy, a guarantee of a medical outcome, or a promise that a third-party payer will contribute. Questions about a fee are handled by email, not by a published telephone line.
Acceptable use
You may use the site to learn, to request care, and to manage your email preferences. You may not use it to break the law, to harass staff, to upload malicious code, to scrape the site in a way that degrades it for others, to misrepresent your identity, or to submit another person’s health information without authority.
You may not copy clinician portraits, layout, or journal text for your own commercial clinic site. Sharing a link to a Neocpm page, including with the share controls in the footer, is welcome. Quoting a short passage with credit for private study is fine. Republishing an article as your own is not.
We may remove a comment, ignore a form, or block further messages that breach this section. We will still act on any content that suggests someone is in immediate danger by directing attention to emergency services rather than by offering treatment through the website.
Intellectual property
The Neocpm name, the heartbeat-and-cross logo, page design, and original text are owned by Neocpm or used with permission. Clinical photographs on the site are used to illustrate real healthcare settings. They are not a statement that every person pictured is a Neocpm employee, and they may not be lifted for advertising.
Trademarks of other organisations that appear incidentally in photographs of real equipment or rooms remain the property of their owners. Their presence does not imply endorsement of Neocpm or by Neocpm.
Email notes, subscribe, and unsubscribe
The subscribe page and the short email fields on the contact page and in the footer add you to occasional care notes: teaching pieces, changes to clinic hours, and reminders about prevention themes. They are not a drip of unrelated advertising. Each signup is confirmed only after the form is validated in your browser; we still rely on you to type an address you actually use.
You may leave the list at any time on the unsubscribe page, or by using a link we place in a note we send. Unsubscribing from notes does not cancel a confirmed appointment and does not close a clinical record. Those are separate requests, made through the contact form or info@neocpm.com.
Please do not subscribe someone else unless you are authorised to receive mail for that address and they expect the notes.
Links and sharing tools
Footer icons open a share action for the page you are on, using services such as a social network or your own email program. Those services have their own terms. Neocpm does not control them and is not responsible for how they display a link. We do not ask you to log in to Neocpm with a social account, because this site does not use accounts.
If a journal note mentions a general concept that you later research elsewhere, that outside page is not our advice. Come back to a confirmed visit for anything that concerns your own health.
Clinical disclaimer
Health information on this website is provided in good faith to help you prepare for care and to understand how Neocpm works. Medicine changes, and a paragraph written for a general reader can be wrong for a specific body. Never start, stop, or change a medicine because of a journal note.
Outcomes depend on diagnosis, other conditions, and the choices you make with a clinician who has assessed you. We do not promise recovery, pain relief, a normal test, or that surgery will be avoided or recommended. Statistics on the home page describe how the service is organised — specialties, clinicians and nurses, clinic days, and a single shared record — not a success rate.
Limitation of liability
To the extent the law allows, Neocpm is not liable for loss arising from your reliance on general website text, from a delay in reading email, from an unconfirmed appointment request, or from a third-party share service. Nothing in these terms excludes liability that cannot legally be excluded, including liability for death or personal injury caused by negligence, or for fraud.
Where liability can be limited, and where a confirmed clinical service is not the subject of the claim, our liability in connection with the website itself is limited to the amount you paid Neocpm for website-related coordination in the three months before the claim, or nothing if you paid nothing.
You are responsible for keeping access to your email account, because that is how we reply. We are not responsible for a mailbox that rejects messages or for a typo in the address you submitted.
Changes to the service and to these terms
We may update hours, clinicians, prices, and these terms as the practice evolves. The date at the top of this page will change when we do. The version you see when you submit a form is the version that covers that submission. Material changes to how we handle health information are also described in the Privacy Policy.
Continued use of the site after an update means you accept the new terms for future use. A visit already confirmed keeps the clinical and fee terms stated in the confirmation email, unless you and Neocpm agree a change.
How to reach us about these terms
Questions about these terms, a fee, or a request you have already sent go to info@neocpm.com. Please use a subject line that says “Terms” or “Appointment” so it is sorted correctly. We do not publish a street address or a telephone number for the practice. Email is the contact path this website offers.
If a law that applies to you requires a particular form of notice, email to that address satisfies our side of website communications unless that law says otherwise. We will reply to the address you write from, unless you ask us to use a different one and we can confirm it is yours.
